A coronial inquiry has commenced into the circumstances surrounding the death of Kate Manley, a 46-year-old woman who died at Albury Hospital in November 2022.[1] Manley was admitted to the hospital's acute mental health ward, Nolan House, as an involuntary patient on November 11, 2022, and died five days later on November 16 at approximately 2 a.m., with a suspected pulmonary embolism identified as the cause of death.[1]
The inquest, presided over by Coroner Rebecca Hosking, commenced on July 31, 2026, at Albury Local Court and is expected to run for two weeks.[1] The inquiry will examine whether Manley received adequate care and treatment during her hospital stay, and whether that care complied with relevant hospital policies and clinical guidelines.[1] A key focus will be hospital protocols requiring a venous thromboembolism risk assessment to be completed within six hours of admission—a procedure that was not in use at Nolan House at the time of Manley's death but has since been implemented.[1] Medical practitioners have raised concerns that Manley did not undergo pathology testing prior to her admission.[1] Throughout her entire hospitalization, Manley was not fitted with anti-thrombotic compression stockings and was not diagnosed with any blood clotting complications.[1]
