Australia's Commonwealth Ombudsman has launched a critical investigation into the Integrated Assessment Tool (IAT), revealing that the automated system used to evaluate aged care needs contains significant defects and produces unfair outcomes.1 Since its rollout on November 1, 2025, the tool has resulted in incorrect care assessments for over 1.1 million older Australians accessing aged care services through the home support system.1
A major concern identified by the Ombudsman is the legal constraint preventing human intervention in IAT decisions. Commonwealth Ombudsman Iain Anderson stated on Monday that "under current legislation, automated decisions made by the IAT algorithm cannot be overturned or changed, even when they are clinically inappropriate."1 The investigation also discovered that no public information exists explaining how the IAT determines its outcomes.1 Adding to the controversy, the Department of Health previously made incorrect claims to assessment providers and the public that IAT results could be modified, a position only corrected when legislation was introduced in October 2025.1
The Ombudsman has put forward 10 recommendations to address the system's failures, including legislative amendments that would allow human override of automated IAT decisions.1 In September 2024, The Guardian reported that departmental staff had warned the health minister of concerns just three days before the IAT's launch.1
