The Irish government formally apologized to the family of Brian Stack, a senior prison officer killed by the Irish Republican Army more than four decades ago.1 Prime Minister Micheal Martin delivered the apology in parliament, addressing the serious investigative failures that undermined the case.1
Stack, who served as chief prison officer at Portlaoise Prison in Dublin, was shot in 1983.1 The attack left him paralyzed, and he died 18 months later at the age of 49.1 The IRA acknowledged responsibility for his killing in 2013.1
A 2007 Garda Síochána serious crime review uncovered 194 errors and omissions in the original investigation.1 These failures included the loss of crime scene maps, failure to preserve a motorcycle helmet, missing fingerprints and other evidence, and the failure to interview prison colleagues, inmates, and Stack's family members.1 Martin told parliament: "What is clearly completely unacceptable is that material now potentially evidential to the investigation cannot be traced by An Garda Síochána."1 In 2019, Garda Commissioner Drew Harris had previously apologized for the mishandling of the investigation.1
