Recurrent mouth ulcers affect approximately one-quarter of the population, with a variety of underlying causes ranging from minor trauma to systemic disease 1. While about eighty percent of ulcers are classified as minor and typically heal within two weeks on their own, more severe cases—particularly major ulcers—can take weeks or even months to resolve 1. In extreme cases, herpetiform ulcers may present as clusters of up to one hundred small lesions 1.
The development of recurrent mouth ulcers is often multifactorial. Potential triggers include local injury to mouth tissue, abnormal immune responses, nutritional deficiencies such as iron, vitamin B12, vitamin C, and folate, and underlying medical conditions including celiac disease, inflammatory bowel disease, lupus, and Sjögren's syndrome 1. Approximately half of those experiencing recurrent ulcers report a family history of the condition, suggesting a genetic component 1. Certain medications—including chemotherapy drugs, nonsteroidal anti-inflammatory agents like naproxen and ibuprofen, some antidepressants, and cardiac medications—have also been associated with ulcer formation 1.
Individuals should seek medical evaluation if an ulcer persists beyond two weeks, as prolonged lesions warrant examination to rule out malignancy 1. Additional warning signs requiring professional assessment include unusually large ulcers, lumps, unexplained bleeding, numbness, difficulty swallowing, voice changes, and unintended weight loss 1. For self-care management, salt water rinses performed for thirty seconds multiple times daily, avoidance of irritating foods, use of a soft toothbrush, and consideration of medicated gels may help alleviate symptoms 1.
