The World Health Organization has released its first global pulse survey documenting the impact of the COVID-19 pandemic on health service delivery worldwide 1. Based on responses from 105 countries covering the period from March to June 2020, the survey reveals that 90% of nations experienced disruptions to essential health services 1. On average, countries reported interruptions in 50% of the 25 tracer services monitored 1.
The disruptions span multiple critical areas of healthcare. Routine immunization outreach services faced a 70% disruption rate, with facility-based immunization services disrupted in 61% of countries 1. Diagnostic and treatment services for non-communicable diseases were interrupted in 69% of countries, while family planning services were disrupted in 68% 1. Mental health treatment services experienced a 61% disruption rate, and cancer diagnosis and treatment services were affected in 55% of countries 1. Emergency services also suffered significant impacts, with 24-hour emergency room services affected in 22% of countries, emergency blood transfusions disrupted in 23%, and emergency surgery services interrupted in 19% 1.
The survey further documents a broader shift in healthcare-seeking behavior and service provision patterns. Seventy-six percent of countries reported declines in outpatient visits, while 66% reported canceling elective services 1. However, only 14% of nations suspended user fees during this period 1. WHO Director-General Tedros Adhanom Ghebreyesus stated: "COVID-19 should send a message to all countries that health is not an 'either/or' equation. We must be better prepared for emergencies while continuing to invest in health systems that fully meet the needs of people throughout their life cycle" 1. The organization plans to introduce learning and monitoring tools to help countries improve health service provision during health emergencies 1.
