The U.S. National Highway Traffic Safety Administration (NHTSA) has launched a federal investigation into Comma.ai's hands-off driving assistance technology following multiple reported accidents.1 The agency's Office of Defects Investigation is examining five crashes associated with the system, two of which proved fatal and resulted in three deaths.1 Additionally, four of the reported incidents caused injuries to eleven people.1
According to NHTSA, the accidents share a common pattern: Comma.ai's equipment failed to detect or respond appropriately to slow-moving or stationary vehicles in the same lane.1 One fatal crash occurred in February 2026 in Ascension Parish, Louisiana, when a 2022 Toyota RAV4 struck a stopped police vehicle, killing two rear-seat passengers.1 That particular vehicle was running FrogPilot, a third-party fork of Comma.ai's openpilot software, suggesting that at least one fatal incident involved a modified version of the company's original system.1 Comma.ai has acknowledged on its website that its adaptive cruise control performance may be affected by stationary vehicles remaining in the same lane.1
