An independent public inquiry released findings on Tuesday concerning Lucy Letby, a nurse convicted of killing seven infants and attempting to murder seven others at the neonatal unit of Chester's Countess of Chester Hospital in northwest England.1 The Thirlwall Report found the hospital guilty of "complete failure" in protecting babies, determining that three infants might have survived and seven others could have been protected had systems functioned properly.1 The crimes occurred between June 2015 and June 2016, with Letby removed from the neonatal unit in July 2016.1
The inquiry identified significant communication breakdowns between hospital leadership and clinical staff, with parents kept in the dark for years about the dangers their children faced.1 The report issued 14 recommendations aimed at preventing future failures, including installation of infant monitoring equipment on all cots and incubators, as well as surveillance cameras around insulin refrigeration units.1 NHS England has been ordered to develop an implementation roadmap for these measures by March 31 next year.1
Letby currently serves 15 life sentences for her convictions.1 The Criminal Cases Review Commission is conducting its own investigation into whether there exists a "real possibility" that her convictions should be overturned.1 When asked about the report, CCRC Chair Vera Baird told the BBC: "We will examine the report with great interest to assess whether it has any bearing on our case review."1 The Thirlwall inquiry was first announced in September 2023.1
