Japan is facing a dual health crisis as syphilis infections continue to climb while supplies of the primary treatment medication dry up. The number of confirmed syphilis cases has more than doubled over the past five years, rising from approximately 6,000 in 2020 to over 13,000 in 2025.1 This surge coincides with a critical shortage of penicillin-based medications, the first-line treatment for the infection.
The drug supply crisis stems from production disruptions at manufacturer Meiji Seika Pharma. The company's treatment product was recalled in July 2025 due to foreign material contamination, followed by a production equipment malfunction in November.1 As a result, the company does not expect to resume shipments until the second half of 2026.1 The shortage carries particular urgency for pregnant women, as penicillin remains the only recommended medication for preventing mother-to-child transmission of syphilis.1 In response to the crisis, a medical society released guidance in August calling for prioritized allocation of available penicillin supplies to pregnant women and patients unable to use alternative medications.1
